Box Lid Fell to Deck

Summary
Whilst helicopter was lifting off an unsecured fibre glass lid was blown off the top of accommodation block. The lid fell approximately 50 feet to the main deck below. Tool pusher was on main deck approx 15 feet away. No other personnel were near.

Description
Location: Ensco 100 at Huntington
Description: Whilst helicopter was lifting off an unsecured fibre glass lid was blown off the top of accommodation block. The lid fell approximately 50 feet to the main deck below. Tool pusher was on main deck approx 15 feet away. No other personnel were near.
The object was 64 cm x 55 cm square in size and 4 cm high.
It weighed = 2.4 kgs
Lessons Learnt
Direct Causes
1. Lid was not securely fastened to Fire & Gas batteries box
Underlying Causes
1. On inspection the lid did have clips to lock it in place but one was not working correctly and the other one was missing the clasp.
Recommendations
Immediate Actions:
1. Conduct hazard hunt to identify any other potential dropped objects
Mitigation Measures:
1. Increase vigilance/inspection criteria on storage boxes/containers
