On the 12th April 2007 the anchor handling tug supply (AHTS) vessel ‘Bourbon Dolphin’ capsized during anchor handling operations West of Shetland with the loss of 8 lives. Following the accident the Marine Safety Forum (MSF) established several workgroups with a view to improving anchor handling procedures. This is just one idea of a tool to help implement PRfS within your own company. You should be aware that copies of these documents ARE NOT available from Step Change, but Apache are happy for you to take them from the list and modify them for your own use.

A recommendation issued by hose manufacturers, based on information gleaned from a number of previous incidents, a series of recommendations for the safe and correct use and maintenance of Bulk Hoses.
These guidelines define the standard dimensions, stacking mechanisms and safe handling operations for cargo baskets 10 metres or greater in length. The guidelines were posted for a consultative period to gather feedback from the industry prior to the final publication. A new procedure and practice has been introduced to improve the safe handling of outbound and inbound cargo. Industry statistics currently reflect an increase in dropped or potential dropped objects from loads. People particularly at risk are yard employees, supply vessel crews, deck crews and the general public. The new process requires the checking and verification that a load is "safe" before it is allowed to be moved.The system applies to outbound loads prepared by vendors/suppliers and to inbound loads from offshore.
