• IP was focused on lifting operation;

• Requirement to close safety bar was considered but not done;

• Ladder safety bar does not have a self-closing mechanism;

• Ladder does not have back protection;• Oil drums storage position not appropriate

Incident Recommendations

•Fit self-closing mechanism to gate

•Replace all safety drop bars with self-closing mechanisms

•Fit back protection to ladder and any others found to have been missed in previous upgrade projects

•Ensure no materials stored in way of access to ladders
