Description of Process

Falls chain inspection conducted on 6 monthly intervals by competent inspector to PEWER standard.

Description of Incident

TEMPSC fall chain noted as failed during routine planned inspection. Fall chain failure due to hydrogen embrittlement after prolonged exposure to adverse environment.

Good Practice Guidance

Falls chain to be inspected to LOLER and have defined replacement period not exceeding 5 years.
