Incident Recommendations

1. Installations & Vessels should set up a maintenance routine to check the performance of the helideck monitors periodically, say once a month. This could be done during a helideck fire exercise!

2. HLO's should be made aware of the need for monitors to meet a certain performance standard and warned to look for any deterioration in performance. It is interesting to note that in every instance the helideck staff reported that the monitors were working satisfactorily the previous weekend. This suggest staff may not be looking for the right performance requirement.

3. Installations/Vessels should review their weather policy for helicopter operations in light of the performance of their monitors and consider curtailing operations sooner than the recommended guideline.
