No formal procedures in place for pressure tesing verification of the hot stab

Procedures had insufficient chacks and balances

Incident Recommendations

Reduce HP riser pressure requirement to 1500 psi, to accomodate potential thermal expansion and agreed with riser supplier and connector manufacturer.

Ensure testing prtocol for verifying the flow of fluid feed from hot satb is in place.

Ensure ROV operator is able to monitor pressure within hot stab system at all times.
