Ensure that a full and adequate risk assessment has taken place prior to task commmencement and that this potential hazard has been recognised, recorded and eliminated.

Incident Recommendations

In the case of DCB type mudskips the slings should be placed on top of the unit with the master link hanging over the side furthest from the direction of fork lift truck appraoch wherever possible. This minimises the risk of the lifting sets becoming snagged.Reinforce that all personnel stay well clear of forklift truck operations, specifically when loading and offloading.
