Lessons Identified

All rigging risk assessments require to consider the risks involved in moving equipment to and from the worksite and storage of equipment at the worksite. Although toe boards were in place around the scaffold edges, the gap at the access point was an uncontrolled hazard in terms of dropped objects.Poor housekeeping, placing the pull lift on top of an accumulation of slings, resulted in a high potential, near miss incident.

Incident Recommendations

The chain of pull lifts should be wrapped and secured, to reduce the risk of unravelling, during transport.
