Lessons Identified

Lesson Learnt #1 - Identify potential dropped objects at equipment design stage and ensure personnel carrying out PUWER assessments of equipment are trained in dropped object prevention.

Lesson Learnt #2 - A Hazard Hunt conducted on the installation identified similar exhaust types on the rig with the same dropped object potential for the rain cap. Equipment with similar rain cap arrangements included crane, air compressors and engine units and will be fitted with secondary retaining devices.

Incident Recommendations

Recommendation #1 - Secure rain caps to exhaust ports on equipment and attach secondary retention.

Recommendation #2 - Check all onshore and offshore equipment for similar rain cap arrangement and take action to ensure the cap is secure and has secondary retention.

Recommendation #3 - Alert equipment suppliers with similar rain cap arrangements to take action to ensure caps are secure and have secondary retaining devices.
