Check sheets for equipment do not specifically mention hoses and nozzles. Risk assessement for the use of this equipment should include the potential for failure of this nature. Maintenace of the unit should include close inspection of this part of the equipment.

Incident Recommendations

Quarantine nozzles until manufacturer completes investigation. Issue industry wide safety alert. Although the operatives Facial Protection Equipment prevented a more serious injury an Investigation will be undertaken to find facial protection that further enhances the operator’s protection.
