Description of Process

Everybody thought that receiving Safety Alerts was important and was sure that Somebody would continue to submit them, so that Tradebody could then distribute them. It’s simple and anonymous and Anybody could do it, and Everybody agrees that they should be doing it. But sadly, Nobody did it.

Description of Incident

Everybody got angry and Somebody complained that they weren’t receiving Safety Alerts and that it must be Tradebody’s fault, but failed to remember that it was Everybody’s job. Everybody thought Somebody should do it, and Anybody knows Everybody should do it, but sadly Nobody realised that Everybody wasn’t doing it, until Somebody missed it.

It ended up that Everybody blamed Somebody when Nobody did what Anybody could have done and Everybody should have done
