Lessons Identified

New worker released the restraining straps and attempted to put sling into crane hook.The individual did not recognise the hazard he was creating, especially that of putting himself into the path of the BOP if it were to topple.

Incident Recommendations

Secure all unstable equipment.New workers to be made aware of job site , tasks and potential hazards.Supervision to monitor the work area more closely.Personnel need to be aware of potential pinch points and areas where equipment has the potential to fall of swing into.Importance of communicating safe practice, via TBT, JSA or similar.
