Description of Task: 

During normal production operations, the crude export meter-stream was lined up to flow through the meter-prover. Shortly after, a work party in the area identified what they thought was a diesel smell and proceeded to the area to investigate. A crude-oil leak was identified and the CCR was notified. A full production shutdown was initiated and crude export stopped.

Lessons Identified: 

The seal manufacturers recommend  a max-life of 3 years and a service-life of 2 years within the 3 year max.

Recommendations: 

Ensure that suitable and sufficient maintenance routines are in place for meter proving loop door seals which are in line with seal manufacturers recommend  max-life.
