Gas release from pressure control valve/pressure regulator

Description of Task: 

During normal production operations, three gas heads activated on low level gas detection in the gas compression cooler deck. The source of leak was located at the export gas analyser let-down cabinet. Initial observation inside the cabinet identified icing on the vent port of a PCV actuator and an isolation was immediately applied outside the cabinet which caused the leak to cease. Further investigation of the PCV revealed that the leak path was through the vent port to atmosphere.

Lessons Identified: 

Should the diaphragm within the PCV fail, the result is a release to atmosphere via a vent on the valve. The manufacturer’s guidance does not state that this is the case.


PCV’s on HC systems should be checked to ensure any vents are located to safe areas.
