A member of a fabric maintenance crew working within a module on a mezzanine deck area heard, smelt and reported a gas leak emanating from the location of the Injection header inlet block valve on the main deck below. Operations technicians were mobilised to investigate and identified the origin of the leak to be from the valve closure stud on the body of the valve at the 5 o'clock position.
Immediate corrective action was taken to initiate the ESD valves and isolate associated flow lines and segregate the area.
HP vents were the opened to depressurise the header and make safe the process.
Investigation findings identified the valves' Internal O Ring seal had failed.
Investigation into industry events with similar failure was to be implemented.
Further maintenance and testing on the valve was to be conducted.
