Description of Process: 

Reciprocating gas compressor with small bore tubing for instrument connections. 

Description of Incident: 

Fatigue failure of instrument tubing following a reinstatement of an incorrect orientation using incorrect fitting practices.

It is important that people are aware of causes of previous failures which could re-occur.

Comments

It should be noted that the use of helical coils on a system where there is significant high frequency vibration adds mass which will add to the pre-stress on the tubing. The use of helical coils in such an application should be avoided. For advice on the installation of tubing in HF vibration applications, the Energy Institute "Guidelines for the design installation and management of small bore tubing assemblies: Annex H Identification & mitigation of dynamic loads & vibration" should be consulted.
