Small weep of gas found coming from 10flexible vent hose

In preparation for the annual shutdown and during a controlled venting operation the platform production technician noticed a slight smell of gas around the platform bridge area. At first due to the still weather conditions he thought it was coming from the vent stack, however on further investigation using a gas tester in the area found a small leak coming from the braided flexible vent hose. The platform fixed gas detection did not pick up any gas due to the small volume. The controlled venting commenced at 00:20 with the starting pressure 14psig, when the smell of gas was noticed the pressure was at 3psig.

Lessons Identified: 


Items not able to be inspected need a definitive replacement regime in place

Recommendations: 

  • Ensure that the flexible hose registers are comprehensive
  • Develop a Flexible Hose Management plan following review of the Management of Flexible Hose Assemblies Guidance (Energy Institute/Oil & Gas UK/HSE)
  • Immediate review of the Flexible Hose register to identify High risk hydrocarbon hoses and those that have passed the guidelines replacement date
  • Identify all due dates for hose replacements and include in Maintenance management system to control and manage hose change-outs
